We are conveniently located on Chorley New Road opposite the war memorial. We have our own client car parking to the rear of the surgery. If you have any trouble finding us, please give us a call, we can give you directions.
Feel free to contact us using the form below and we’ll respond as soon as we can. Unfortunately we cannot take prescription requests or book appointments using this form. This is so we can ensure we are giving you the correct advice and taking all necessary information. Please phone us on 01204 525066 for appointments and prescription requests.
